Patient Forms

Upon entering our office, our receptionist will  welcome you as a member of our family.  We will request that you complete our  patient forms located in the New Patient Center. This paperwork provides us with your health history and information on your condition.


Next, you will have a consultation with Dr. Kenny Sheppard to discuss your health-related problems, concerns, and potential treatment options. This initial visit is designed for Dr. Kenny Sheppard to learn more about you, your condition, and  expectations to determine how upper cervical chiropractic care can meet your goals.


After your consultation, Dr. Sheppard will perform a chiropractic examination. This exam includes neurological, orthopedic, postural and specialized tests to determine the extent of your spinal imbalance.

X-Ray Studies

A radiographic study of your spine allows the doctor to identify your spinal condition and alignment. He will take a specific three view cervical x-ray series on a digital x-ray machine to determine the exact amount of spinal misalignment. This is a specialized precision lined x-ray system that is unique to our practice. We may also take additional x-rays of other spinal areas as needed.

Report of Findings

Once the information is collected  and examinations are performed, Dr. Kenny Sheppard will give you a detailed report of all findings and answer any questions including:

  1. How can you help me?
  2. How significant is my spinal misalignment?
  3. How often do I need to come in?
  4. What  will  my treatment  cost?

After reviewing your health history,  goals, and examining your spine and X-rays, Dr. Kenny Sheppard will discuss recommendations and notify you if  your condition  requires care with other providers.  We believe in a team approach and will  provide the best treatment program for your needs.


Our goal is to return your spine to a normal balance. A very specific and gentle force moves the Atlas vertabrae back into its normal position. The patient usually feels nothing but a slight pressure from the doctor's hand during the adjustment.
